Lake Metroparks welcomes you to camp! The following information will help to assure that your child will have a safe and enjoyable camp experience.
Required Camp forms

If you registered your child for extended care, download and complete the Extended care agreement form for your camp and bring it with you on the first day of camp.

If the camp you registered your child for includes a field trip or is equine related, download and complete the following waiver form(s) indicated in your confirmation email and bring with you on the first day of camp.
